We welcome anyone looking for cancer care or information about his or her cancer risks.

What Should I Bring to My First Visit?

To give you the best care, it’s helpful for us to learn as much as we can about you.

We appreciate any information you can provide about your health history and any tests, procedures or treatments that you’ve had before.

On your first visit to the Ralph Lauren Center for Cancer Care, it’s helpful to bring with you the following items:

Please note: If you do not have some of the items listed below, or have questions about what to bring, we can help. Please call us at (212) 987-1777, and we will be happy to assist you.

  • Photo ID

    Any form of photo identification, such as your New York State driver’s license, New York State identification card, passport, etc.

  • Insurance card(s)

    If you are covered by more than one insurer, please bring both cards.

  • Doctor referral and/or insurance referral from your primary care doctor

    A note from your doctor that recommends or authorizes you to visit the Center.

  • Recent medical reports and summaries

    Descriptions written by your doctor that details your medical problems and treatments you have received.

  • Radiology reports

    Tests taken to produce images of the tumor(s). Images may be on disc or film. Examples include X-rays, CT scans, MRIs, PET scans and bone scans.

  • Pathology reports

    Glass slides that contain tissue samples from the tumor(s).

  • List of medications (if applicable)

    Please be sure to bring the names of all medications you take, including those prescribed by doctors and those you buy over the counter. This list should include any vitamins or herbal products you take.
